By checking this box, I hereby acknowledge that it is my desire for my child to participate in the church sponsored activities at Orange Beach Methodist Church, including activities on and off church property. My child is voluntarily participating in these activities, and I acknowledge the dangers involved. I hereby agree to accept any and all risks of injury as a result of such participation. I hereby release and discharge Orange Beach Methodist Church, its officers, employees, agents, volunteers, and members of the Leadership Counsel from all liability from losses or damages that may result from participating in this program. I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability.
Transportation Release Consent Agreement
At times it becomes necessary to use private vehicles or the church-owned van to transport students to and from Orange Beach Methodist Church for children/youth activities. When this occurs, Orange Beach MC requires that a parent/guardian of the student sign the Transportation Release Consent Form that appears below.
By clicking agree in this section, I hereby release Orange Beach Methodist Church, its officers, employees, agents, volunteers, and members of the Leadership Counsel from all liability from losses or damages and all injuries arising while traveling to activities via private vehicle or church van.
In the event of an accident or injury, if a ministry leader of Orange BeachMethodist Church determines that 911 should be called, I give permission for Orange Beach MC to obtain emergency medical treatment, including emergency transportation, for my child if I cannot be reached immediately. I agree to be responsible for any emergency medical expenses incurred. (If parent/guardian refuses to sign, instructions must be submitted below stating what procedure Orange Beach Methodist Church is to follow in an emergency)
By registering your child for any activities at Orange Beach Methodist Church please understand that your child may be photographed. These photos may potentially be used on our Church Facebook or website to promote other Children's/Student Ministry programs of OBMC. Names of persons pictured will not be used without consent.
